Note:

The deadline for registration at organization committee is April 30, 2018. Overdue deliverables will be regarded as a waiver. Each delegation shall send annexes from annex one to annex eight above to the mailbox designated by the executive by email or fax. Also, the above annexes will be delivered by mail, attached by photograph for each participant and one copy of identity certificate.

Annex Six: Safety Responsibility Commitment Letter

The 15^th 2018 World Chin Woo Martial Arts Cultural Competition

Declaration of Safety Responsibility

Delegation:                           Total                     athletes. 

The leader of each delegation shall read and inform his/her athletes to understand and agree to be bound by the following matters:

1. Clearly understand that athletes must bear full responsibility for any accidental casualties. Clearly understand that all medical attention or treatment offered by organizers will be the first aid only. Athletes are responsible for any accident during the emergency treatment.

2. The organizers and sponsors do not assume any responsibility for any accidents or disasters during the competition.

       3. Ensure that athletes do not intake any medicine (stimulant) or drug

       4. Ensure that athletes have not participated or been involved in any illegal activities.

5. Ensure that athletes are physically and mentally healthy as well as suitable to attend the competition

6. Athletes should keep their own personal belongings and valuables. Organizers and sponsors are not responsible for any loss, theft or damage during the competition.

7. Athletes agree and follow the regulations or rules established by the Chinese government and organizers. And protest must be conducted in accordance with the rules of Arbitration

8. All athletes' exercise, competition and other activities in the Competition may be filmed, recorded, or lived in telecast. Athletes agree with the Organizing Committee to use athletes’ names, addresses, voices, poses, pictures and biographical data concerning full or parts, in any form or any language, with or without other materials, throughout the world, without limitation, for television, radio, video, medium picture, or any other media by any devices now known or hereafter. Athletes do hereby waive any compensation or any future rights to the above mentioned items
